Some people can do any type of physical activity. Other people need to avoid certain types of physical activity. Please answer the following questions to let us know what types of physical activity are appropriate for you.

Are you able to:

  1. Play competitive sports or on rep/travelling teams?
  2. Play recreational sports or on house league teams?
  3. Do activities where people intentionally bump into each other?
  4. Do activities where people often bump into each other but it is not intentional?
  5. Do activities that make your breathing go as fast as it can?
  6. Do activities that make your breathing go a little faster?
  7. Do activities that make your heart beat really fast?
  8. Do activities that make your heart beat faster?
  9. Do activities where you suddenly move really fast (e.g., sprinting, basketball)?
  10. Push or pull as hard as you can?
  11. Push or pull so hard that you hold your breath?
